A signal from an irritated tissue does not travel straight to your awareness. It has to hand itself off at a checkpoint in your spinal cord first — and your brain gets a say in how loud that handoff is.

Running down from your brainstem to that checkpoint is a network that turns the incoming signal up or down before it reaches you. Researchers call it the descending pain modulatory system. Plainly: your nervous system decides how loud the report gets played.

This is well-established science. The Journal of Orthopaedic & Sports Physical Therapy has published a clinician's guide to this system for physical therapists, precisely because it explains so much of what happens in a treatment room.

Two Ways to Turn the Volume Down

The useful part is that this system has two separate front doors.

From the body up. Certain input from your tissues shifts the system toward quiet — hands-on work, movement, pressure, temperature. This is the published mechanism behind why manual therapy and the right kind of exercise reduce pain. It is a physical circuit being engaged from the outside in.

From the brain down. Expectation, understanding, attention, and the relationship you have with the person treating you feed the same network from above. When researchers say the appointment itself reduces pain, they mean a measurable physical effect, not a figure of speech — the same machinery that pain medication targets, switched on by context.

Which is why an unhurried appointment where someone explains what they found is not a courtesy. It is part of the treatment.

The Same System Can Turn Pain Up

This network runs both directions. Under stress, poor sleep, or a body that has been guarding for months, the balance shifts and the same circuit starts amplifying instead of dampening. Less input, more pain. Nothing about the tissue changed — the volume control did.

None of this means your pain is imagined. The volume control is physical, and so is the tissue it is reporting on.

That shift is a defining feature of long-standing pain. It is why a scan can be unremarkable while the pain is very real. Why This Shows Up in Your Muscles

The brainstem regions that run this pain system share their neighborhood with the circuits that set muscle tone. Different jobs, same part of the nervous system.
